Folder contains 9 documents labeled by Hasuike as "Family Documents." Each relate to members of the Ochiai family. Document 1 is a union dues booklet from the International Association of Machinists. It was issued to son Kazuo Ochiai in May 1955. Ochiai is listed as a "Production Worker" most skilled at "Classfication." Document 2 is a Designation/Change of Beneficiary notice for Kazuo Ochiai dated June 6th, 1941. The document is issued by the Railroad Retirement Board. It designates Kazuo's father, Zenzaburo, as the beneficiary of any death benefits. Kazuo is listed as a "Laborer" under the Operating Division of the Spokane, Portland, and Seattle (SP&S) Railroad Company. Document 3 is the birth certificate of daughter Shizuko Ochiai (born at 9:30 AM on November 12th, 1926) from the Portland Bureau of Health mailed to midwife Takako Urakami. Shizuko was born to parents Zenzaburo and Tamiyo. Document 4 is a notice dated October 15th, 1948 from the Selective Service System ordering Kazuo Ochiai to report for physical examination at the US Army and Air Force Recruiting Station. Document 5 is father Zenzaburo Ochiai's 1919 Japanese passport. It was issued in December of 1919 and allowed Zenzaburo to return to Japan from the United States, and again to the United States. The document folds out, and the interior contains a photo of Zenzaburo. Document 6 is the smallpox vaccination record for mother Tamiyo Ochiai dated January 24th, 1970. Documents 7-9 are three 1941-2 report cards for Shizuko Ochiai from Lincoln High School. The first records the term from January-June 1941, the second September 1941-January 1942, and the third January-June 1942. The third is incomplete, most likely due to the her forced relocation with the rest of the Japanese American community in 1942. |
